Forms You Need to Complete
Before we can provide the safest, most appropriate treatment, we need to learn about details like your medical history and the medications you take. Visit our forms page to access our patient registration form. It’s easy to complete the form online, but if you’re not able to do so, please plan to arrive 15 minutes before your appointment so we can get you registered.
When You Arrive at Our Office
We’ve designed our reception area so that it looks more someone's comfortable living room. When you arrive, our front office staff will greet you by name and sign you in. During your short wait, help yourself to an assortment of Keurig beverages while you browse our current selection of magazines.

Meeting Your Dental Team
A member of our team will show you to your treatment room to get settled. You will meet one of our friendly, helpful dental hygienists who will spend a few minutes chatting with you about any concerns you have. She will check for signs of gum disease or other areas of concern and provide a comprehensive dental cleaning.
After your cleaning, you will meet with one of our dentists who will talk to you about any concerns we have and how treatment will benefit you. They will answer all your questions because we want you to feel informed when you make decisions about your oral health care. Together, we’ll develop a plan for treatment to correct any problems and restore your healthy smile.
Before you leave, a member of our team will help you set up future appointments and review your insurance benefits and how they apply to your treatment.
